Permission confirmation buttons do not work in Codex (web and mobile)

Title: Permission confirmation does not work in Codex UI (web and mobile)

Description:
In Codex, permission confirmation buttons do not work.

When an action requires permission, the UI shows three buttons (for example: Yes, Yes and don’t ask again, Stop and explain). Clicking any of them only shows a brief loading spinner, but the permission is not applied and the agent does not continue.

This happens both in the Codex web interface and in the mobile interface.
The same workflow works correctly in Cloud, where clicking Yes immediately confirms the permission and execution continues.

I've created a fork where I'm fixing Codex related issues https://github.com/shayne/happy
I've fixed this.

On npm it's happy-codex and you can just npx -y happy-codex@latest to start a new session
