donard.1.13: Vivid Vervet Kernel

Same as previous but based on the Ubuntu kernel.

donard.1.12: This release removes the PFN patches for ib/umem in favour of using the

io_peer_mem module that imploys the peer memory client interface.

It also fixes follow_pfn so we can finally RDMA with buffers from DAX
filesystems.
