Hide markdown mode: Allow replacing markup on all lines when the Markdown editor isn't focused #37

This may involve doing one of the following:
- New option: "Replace all markup when unfocused" that hides or replaces all Markdown formatting when the editor doesn't have keyboard focus.
- This might not need to be a setting?
- New option: "Always focus first line" (or last line?) that automatically moves the cursor to the first line when opening a note.
