"Report repository" feature is not working after selecting report category

Hi!

In the Hungarian developer community a public repository was discovered, that contains ~7.000 people's personal data (name, email address, phone number, date of birth, and some comments from job interviews). I have tried to use the "Report repository" link from the main page of the repository, but after selecting the category of the report, the message text input field, the captcha and the Submit button disappeared. I have tried this from Opera and Edge browsers, from incognito mode. A colleague of mine could report the repository from Chrome. Another colleague experienced the same issue also from Chrome with incognito mode.

I have hidden the repository since the repository is already reported, however it already has 21 forks.

The issue is with the category selected: if the "Data Protection and Policy Violations" category is selected, the page only display a note where the data protection policies of GitHub can be opened. After selecting the "Doxxing or Invasion of privacy" category, the report could be submitted.
