What issue are you seeing?
- Version: Codex CLI 0.53-ish (latest).
- Expectation: registered notify hook should receive approval-requested.
- Actual: approval prompts appear in the UI but only agent-turn-complete
events reach the hook; manual invocation works. - Evidence: ~/.codex/log/codex-apple-voice-notify.log missing entries, plus
manual test output.
approval-requested is never logged.
