Copy over the Nightscout doc page from the v2 docs and update it with newer steps on setting up nightscout.
Note that for some reason there are two different nightscout pages (https://triodocs.org/install/ecosystem/nightscout-app/ and https://triodocs.org/configuration/settings/services/nightscout/), I updated both.
