thinking content flickers with code blocks

this scroll jumping is caused by the markdow renderer re-rendering the items in the thinking part to not have the code blocks, which reduces the height, which causes a didHeightChange to get fired off, which resets the height, then gets reset again.

repro steps:

1. must have thinking/reasoning enabled for anthropic models
2. ask something that might create code blocks in the thinking, like "tell me about this file #file: github.md"
