bug: duplicate model ids can be added

Describe the Bug

Currently, users can add the same remote model ID multiple times. Each duplicate entry still appears in the list

This leads to cluttered lists and confusion, since multiple identical entries exist for the same model. Additionally, when one is deleted, all duplicates disappear at once

Steps to Reproduce

1. Navigate to Settings > Model Providers (e.g., OpenAI).
2. Add a remote model with an ID (e.g., gpt-5).
3. Add the same model ID again.
4. Observe that the duplicate entries appear in the list.
