[bug]: Emdesh creates a `.claude/settings.json` even though I'm using Codex

Bug Description

I'm using and Emdesh creates a .claude/settings.json.

Here you can see that this file was part of my generated changes: SharezoneApp/sharezone-app@38eea19.

Steps to Reproduce

1. Use Codex as agent
2. Enable "Auto-Approve"

Actual vs Expected Behavior

It shouldn't create a .claude/settings.json with

{
  "autoApprove": true
}

Also, I think this could be issue by my auto-approval for Codex isn't working (it still asks me for permission).
