Add extract_pdf error handling tests #4
Description
Add tests that exercise extract_pdf error paths without requiring pdftotext to be installed.\n\nAcceptance criteria:\n- Test raises RuntimeError when pdftotext is missing.\n- Test raises RuntimeError with stderr when pdftotext exits non-zero.
