扫描不到skill但是确实有配置，且配置正确
What did you expect to happen?
描述：
1. 在 ~/.qwen/skills/map-test/ 目录下存在有效的 SKILL.md 文件
2. 目录结构：C:\Users\HP.qwen\skills\map-test\SKILL.md
3. 文件格式正确（UTF-8，无BOM，首行 # map-test - Excel 批量生成工具）
4. 但系统启动时 <available_skills> 显示 "No skills are currently configured"
5. 通过 glob 手动搜索可以找到该文件
