Support "external" ports when reverse proxy is in use

As noted in #232, when a reverse proxy is in place doing OIDC becomes difficult because the constructed callback URL contains the tunnels port and domain rather than the port exposed by the reverse proxy. 

Then `NumberOfProxies` is > 0 we should enable an additional feature on the tunnel to be able to set this.
