Well, it turns out (I haven't tried other games yet, but from 6 to 15 it works great). My problem is that in 19, the mouse is active, but for some reason it doesn't respond. The game just switches from mouse to keyboard even though it's active.

For Touhou 19, ThMouseX supports version v1.10c, the latest version. Are you using this version?
