CI: modernize GitHub Actions (PHP 8.2–8.5) (#23)

* CI: modernize GitHub Actions (PHP 8.2–8.5)

Phase 0 housekeeping:
- Upgrade actions/checkout and actions/cache to v4
- Test on PHP 8.2–8.5
- Remove Scrutinizer coverage upload
- Fail the suite on test errors (no continue-on-error)

* Add doctrine/annotations to require-dev for ORM annotation driver tests

Needed for Doctrine ORM Setup::createAnnotationMetadataConfiguration on modern ORM.

* Disable SimpleAnnotationReader in tests for doctrine/annotations 2

Setup::createAnnotationMetadataConfiguration defaults to simple reader,
which was removed in annotations 2.x.

* Pin doctrine/annotations to ^1.13 for SimpleAnnotationReader in tests

Annotations 2 removed SimpleAnnotationReader; fixtures use short @entity names.
